THE SOCIAL SECURITY ADMINISTRATION NEEDS THE INFORMATION COLLECTED ON FORM SSA-623 TO DETERMINE WHETHER THE PAYMENTS CERTIFIED TO THE REPRESENTATIVE PAYEE HAVE BEEN USED FOR THE BENEFICIARY'S CURRENT MAINTENANCE AND PERSONAL NEEDS AND WHETHER THE REPRESENTATIVE PAYEE CONTINUE TO BE CONCERNED WITH THE BENEFICIARY'S WELFARE.
