THIS FORM IS USED BY DISTILLED SPIRITS PLANTS TO TRANSMIT, TWICE EACH MONTH, PAYMENT OF FEDERAL EXCISE TAXES ON DISTILLED SPIRITS. THE FORM IDENTIFIES THE TAXPAYER, THE AMOUNT PAID AND HOW IT WAS DETERMINED, AN THE PERIOD COVERED.
