Abstract:
SECTION 204(A) OF THE SOCIAL SECURITY ACT AND SECTION 413(B) OF THE FEDERAL MINE SAFETY AND HEALTH ACT PROVIDE THAT IF AN INDIVIDUAL DIES BEFORE BENEFIT PAYMENTS HAVE BEEN COMPLETED, THE AMOUNT DUE WILL BE PA TO PERSONS MEETING SPECIFIED QUALIFICATIONS. THIS FORM IS USED TO DETERMINE WHO IS HIGHEST ON THE PRIORITY LIST FOR PAYMENT.
