THE INFORMATION COLLECTED BY USE OF FORM SSA-L8050-U3 IDENTIFIED SSI APPLICANTS/RECIPIENTS POTENTIALLY ELIGIBLE FOR OTHER BENEFITS, SO THAT THEY MAY FILE FOR AND RECEIVE SUCH BENEFITS.
