THE APPLICATION CARD IS THE BASIC OPERATING DOCUMENT USED IN STATE PUBLIC EMPLOYMENT SERVICE AND WIN LOCAL OFFICES FOR NEW AND PARTIAL APPLICATIONS FOR INDIVIDUALS SEEKING ASSISTANCE IN FINDING EMPLOYMENT OR EMPLOYABILITY DEVELOPMENT SERVICES.
