THE INFORMATION COLLECTED BY THE USE OF FORM SSA-3441 IS NEEDED TO DETERMINE IF A CLAIMANT'S MEDICAL OR VACATIONAL SITUATION HAS CHANGED SINCE THE INITIAL SSA DENIAL DETERMINATION ON THE CLAIM WAS MADE.
