THIS FORM IS USED TO COLLECT INFORMATION ON MINORITY BUSINESS CAPABILITIES FOR REFERRAL TO PROCUREMENT OFFICIALS INTERESTED IN EXTENDING CONTRACT BIDDING OPPORTUNITIES TO MINORITY FIRMS. RESPONDENTS ARE MINORITY OWNERS OF BUSINESS FIRMS CAPABLE OF AND INTERESTED IN SELLING GOODS AND SERVICES TO GOVERNMENT AGENCIES AND OTHER BUSINESSES.
