Abstract:
THE INFORMATION COLLECTED BY THE USE OF THIS FORM IS NEEDED IN ORDER T DETERMINE ONE-HALF SUPPORT, WHICH IS A REQUIREMENT FOR ENTITLEMENT TO PARENT'S BENEFITS. ONE-HALF SUPPORT IS ALSO REQUIRED TO MEET THE EXCEPTION TO GOVERNMENT PENSION OFFSET. THE AFFECTED PUBLIC CONSISTS OF PARENTS OF DECEASED WORKERS WHO CLAIM BENEFITS AND SPOUSES WHO ARE SUBJECT TO GOVERNMENT PENSION OFFSET AND CLAIM ONE-HALF SUPPOR
