THIS FORM IS ISSUED BY BANKS, CREDIT UNIONS, INSURANCE COMPANIES, AND TRADE OR PROFESSIONAL ASSOCIATTIONS TO APPLY FOR APPROVAL OF A SIMPLIFIED EMPLOYEE PENSION PLAN TO BE USED BY MORE THAN ONE EMPLOYER. THE DATA COLLECTED IS USED TO DETERMINE IF THE PROTOTYPE PLAN SUBMITTE IS AN APPROVED PLAN.

On behalf of this Federal agency, I certify that the collection of information encompassed by this request complies with 5 CFR 1320.9 and the related provisions of 5 CFR 1320.8(b)(3).
