THIS FORM IS USED TO CONFIRM INFORMATION SUPPLIED BY NEW LICENSEES REGARDING THEIR STOCKHOLDERS AND TO DETERMINE THESE STOCKHOLDERS' SOURCES OF FUNDS USED TO ACQUIRE THEIR STOCK. THIS INFORMATION IS NEED TO DETERMINE COMPLIANCE WITH THE SMALL BUSINESS INVESTMENT ACT OF 1958, AS AMENDED, AND THE REGULATIONS THEREUNDER.
