THIS FORM IS USED TO COLLECT INFORMATION NEEDED TO MAKE CHARACTER DETERMINATIONS WITH RESPECT TO APPLICANTS FOR ASSISTANCE OR FOR TEMPORARY EMPLOYMENT IN DISASTER OFFICES.
