Abstract:
THIS FORM IS USED BY THE CIVIL SERVICE RETIREMENT SYSTEM TO DETERMINE THE CONTINUED ELIGIBILITY OF UNMARRIED DEPENDENT CHILDREN BETWEEN 18 AND 22 YEARS OF AGE TO RECEIVE SURVIVOR ANNUITY BENEFITS. IT COLLECTS INFORMATION REGARDING MARITAL STATUS, CURRENT FULL-TIME SCHOOL ATTENDANCE AND FUTURE PLANS FOR FULL-TIME SCHOOL ATTENDANCE.
