JOB CORPS ENROLEES MAY ELECT TO HAVE A PORTION OF THEIR READJUSTMENT ALLOWANCE SENT TO A DEPENDENT MONTHLY. THIS FORM PROVIDES THE INFORMATION NECESSARY TO ADMINISTER THOSE ALLOTMENTS. QUALIFICATION FOR THE ALLOTMENT.
