THE INFORMATION COLLECTED BY THIS FORM IS PROVIDED BY CLAIMANTS WHO ARE REQUESTING A RECONSIDERATION OF THE DETERMINATION BY THE SOCIAL SECURITY ADMINISTRATION THAT THEIR DISABILITY HAS CEASED, DID NOT EXIST, OR IS NO LONGER DISABLING.
