THIS REQUEST IS APPROVED FOR USE ONLY FOR THE PROGRAM PERIOD ENDING JUNE |987 UNDER CONDITION THAT THE "DEAR COLLEAGUE" LETTER SENT AS PART OF THE REQUEST ACCOMPANIES THE DISTRIBUTION OF THE FORM. THAT LETTER EXPLAINS THAT THE NEXT REPORT WILL REQUIRE ADDITIONAL DATA ON THE REPAYMENT OF SCHOLARSHIPS. ADDITIONALLY, FOR THIS YEAR THE TITLE OF PART V OF THE INSTRUCTIONS WILL BE CHANGED TO "FULFILLMENT OF SCHOLARSHIP OBLIGATION" TO BE CONSISTANT WITH THE PART V HEADING ON THE FORM ITSELF.
