THE STATE OF PERSONAL INJURY POSSIBLE THIRD PAR LIABILITY FORM IS COMPLETED BY CHAMPUS/CHAMPVA BENEFICIARIES SUFFERING FROM PERSONAL INJURIES AND RECEIVING MEDICAL CARE AT GOVERNMENT EXPENS THE INFORMATION IS NECESSARY IN THE ASSERTION OF THE GOVERNMENT'S RIGH TO RECOVERY UNDER THE FEDERAL MEDICAL CARE RECOVERY ACT.
