THIS FORM IS USED TO DOCUMENT THE ATTENDING PHYSICIAN'S STATEMENT THAT THE HOSPITALIZATION WAS REQUIRED DUE TO AN EMERGENCY AND GIVE CLINICAL SUPPORT FOR THE CLAIM. THIS FORM HAS BEEN IN USE FOR SOME TIME AND IS CURRENTLY APPROVED.
