THE INFORMATION COLLECTED BY USE OF THE FORM SSA-2519 IS NEEDED AND USED TO ENTITLE CHILDREN TO SOCIAL SECURITY BENEFITS UNDER THE DEEMED RELATIONSHIP PROVISION. THE AFFECTED PUBLIC IS COMPRISED OF INDIVIDUALS WHO HAVE INFORMATION RELEVANT TO THE DETERMINATION OF A CHILD'S RELATIONSHIP TO AN INSURED WAGE EARNER.
