THIS APPLICATION FORM PROVIDES INSTRUCTIO AND INFORMATION NEEDED FOR APPLICANTS TO SUBMIT A REQUEST FOR FEDERAL ASSISTANCE. THE INFORMATION SUBMITTED IS USED BY THE OFFICE OF SPECIA EDUCATION TO DETERMINE GRANTEE ELIGIBILITY, ACCEPTABILITY OF APPLICATIONS, AND AMOUNT OF GRANT AWARDS.
