Abstract:
THIS DATA IS NECESSARY TO ESTABLISH APPLICANT'S CITIZENSHIP AND IDENTIFY AND FOR PASSPORT SERVICES TO DETERMINE ENTITLEMENT TO THE ISSUANCE OF A U.S. PASSPORT. THE INFORMATION SOLICITED IS AUTHORIZED, BUT NO LIMITED TO, THOSE STATUTES CODIFIED IN TITLES 8, 18, 22 U.S.C., ALL REGULATIONS ISSUED PURSUANT TO E.O. 11295 AND SECTION 6039E OF THE IRS CODE OF 1986.
