THIS FORM IS USED BY AN EMPLOYER TO MAKE AN AGREEMENT TO PROVIDE BENEFITS TO ALL EMPLOYEES UNDER A SALARY REDUCTION SIMPLIFIED PENSION (SEP) DESCRIBED IN SECTION 408(K).
