THE INFORMATION COLLECTED VIA THE SSA-521 IS NEEDED TO EFFECTUATE AN INDIVIDUAL'S WITHDRAWAL OF A CLAIM FOR SOCIAL SECURITY BENEFITS. THE AFFECTED PUBLIC IS COMPRISED OF INDIVIDUALS WHO DESIRE TO WITHDRAW APPLICATIONS FOR SOCIAL SECURITY BENEFITS.
