THE INFORMATION COLLECTED ON THIS FORM IS USED BY THE SOCIAL SECURITY ADMINISTRATION TO PROCESS A REQUEST FOR A HEARING ON AN UNFAVORABLE DETERMINATION. FORM HA-501 IS USED BY INDIVIDUALS WHOSE CLAIMS FOR SOCIAL SECURITY BENEFITS HAS BEEN DENIED AND WHO REQUEST A HEARING OF THE FACTS CONCERNING THAT DENIAL.
