Abstract:
THE INFORMATION COLLECTED BY THIS FORM WILL ALLOW A QUALIFIED INDIVIDUAL WHO REQUESTS TO BE MADE A SUBSTITUTE PARTY TO PROCEED WITH THE PENDING CLAIM OF A DECEASED CLAIMANT WHOSE APPLICATION FOR SOCIAL SECURITY BENEFITS HAD BEEN DENIED AND WHO HAD REQUESTED A HEARING. THE AFFECTE PUBLIC CONSISTS OF PERSONS WHO WISH TO PURSUE CLAIMS ON BEHALF OF DECEASED CLAIMANTS.
