THE INFORMATION COLLECTED BY THIS FORM IS NEEDED BY SSA TO DETERMINE THE CONTINUING ENTITLEMENT TO SOCIAL SECURITY BENEFITS AND T PROPER BENEFIT AMOUNTS OF BENEFICIARIES LIVING OUTSIDE THE U.S. THE AFFECTED PUBLIC IS COMPRISED OF PERSONS LIVING OUTSIDE THE U.S. WHO AC AS REPRESENTATIVE PAYEES FOR A MINOR CHILD OR AN ADULT UNABLE TO HANDL HIS/HER FUNDS OR WHO ARE REPORTING THEIR OWN CIRCUMSTANCES.

On behalf of this Federal agency, I certify that the collection of information encompassed by this request complies with 5 CFR 1320.9 and the related provisions of 5 CFR 1320.8(b)(3).
The following is a summary of the topics, regarding the proposed collection of information, that the certification covers:
