Terms of Clearance: This information collection is approved for use until May 1994. OMB approval extends to the IHS-856 and the forms contained in the applicant information instruction booklet and the student handbook. The IHS Scholarship application form must be corrected to change the form number from HRSA-856 to IHS-856. IHS must submit a revised burden estimate as soon as possible to include those forms contained in the student handbook and the IHS 815, 816, 817 and 818.
