In accordance with the Paperwork Reduction Act and 5 CFR 1320, this information collection request is approved for use through 8/94. If the Rural Electrification Administration (REA) chooses to resubmit thi information collection request, REA should review the efficacy of requiring borrowers to submit annually an entire form and evaluate the feasibility of allowing borrowers to submit only changes to the information contained on this form on an annual basis.

Abstract:
USED TO OBTAIN UP-TO-DATE MAILING INFORMATION ON PEOPLE WITH WHOM REA HAS OUTSIDE CONTACT ON A RECURRING BASIS. THE INFORMATION IS USED TO ENSURE THAT PROPER OFFICIALS HAVE SIGNED THE DOCUMENTS THAT ARE SUBMITTED TO REA AND THAT REA CORRESPONDENCE IS PROPERLY DIRECTED.
