Approved with the revisions to the form and instructions submitted to OMB on November 26, and November 27, 1991. The revisions will comply with changes required by the terms of clearance extending approval through September 30, 1985. The copy of the form submitted to OMB with this request has an expiration date of December 31, 1989, but the OMB records show that Paperwork Reduction Act approval of the form expired on September 30, 1985. The Department and agency are reminded of the effect of the public protection clause under which no person shall be subject to any penalty for failure to comply with any collection of information that does not display a valid OMB control number (5CFR1320.5(a)).

On behalf of this Federal agency, I certify that the collection of information encompassed by this request complies with 5 CFR 1320.9 and the related provisions of 5 CFR 1320.8(b)(3).
