THIS APPLICATION AND INFORMATION WILL BE USED TO REQUEST AND DETERMINE ELIGIBILITY FOR CERTAIN ACTIONS AFTER ANOTHER IMMIGRATION BENEFIT HAS BEEN ACCORDED. THE APPLICATION WILL BE USED TO REQUEST A DUPLICATE APPROVAL NOTICE, TO REQUEST ANOTHER U.S. CONSULATE BE NOTIFIED THAT A PETITION HAS BEEN APPROVED, OR TO REQUEST THAT A U.S. CONSULATE BE NOTIFIED THAT A PERSON HAS BEEN ADJUSTED TO PERMANENT RESIDENCE STATUS
