We have approved the LS-208 for three years, the maximum period allowe by the Paperwork Reduction Act. Due to infrequent revisions to this fo permission is granted to print the form without an OMB expiration date DOL shall continue, however, to include the burden disclosure statemen required by 5 CFR 1320.
