This information collection is approved through 8-93 under the following condition: SSA will reestimate the burden associated with this submission, as 10 minutes does not accurately reflect the time it would take a respondent to complete the form. OMB recommends that the burden estimate be revised to at least 15 minutes. The burden disclosure statement appearing on the revised form must reflect the new estimate. Additionally, SSA must submit an inventory correction worksheet to OMB reflecting the reestimated burden.

THE INFORMATION COLLECTED ON THIS FORM IS USED TO MAKE A DETERMINATION OF ELIGIBILITY FOR SUPPLEMENTAL SECURITY INCOME (SSI) PAYMENTS. THE RESPONDENTS ARE APPLICANTS FOR SSI PAYMENTS WHO NEED TO ESTABLISH NONDISABILITY ELIGIBILITY REQUIREMENTS.
