INFORMATION IS REQUIRED OF TESTING LABORATORIES VOLUNTARILY APPLYING FOR ACCREDITATION UNDER THE NATIONAL VOLUNTARY LABORATORY ACCREDITATIO PROGRAM. APPLICANTS PROVIDE THE MINIMUM NECESSARY INFORMATION REQUIRE BY PUBLISHED ACCREDITATION CRITERIA TO ACHIEVE EFFECTIVE ON-SITE ASSESSMENTS AND INFORMED EVALUATIONS LEADING TO PROPER ACCREDITATION DECISIONS.
