The primary change suggested by OMB regards a change to the beginning text of the instructions to read as follows: "An original or a certified copy of a court order appointing a guardian or fiduciary to handle the affairs of the individual must be submitted...or other affairs. Uncertified copies are not acceptable. The court order shou cover."
