THE INFORMATION WILL BE USED BY THE INS TO DETERMINE ELIGIBILITY FOR EMPLOYMENT AUTHORIZATION UNDER 8 CFR 274A.12(A) OR (C). THE ISSUANCE EMPLOYMENT DOCUMENTATION WILL ALSO BE BASED UPON INFORMATION PROVIDED ON THIS COLLECTION. ONLY ALIENS REQUESTING EVIDENCE OF EMPLOYMENT AUTHORIZATION WILL BE REQUIRED TO USE THIS FORM.

On behalf of this Federal agency, I certify that the collection of information encompassed by this request complies with 5 CFR 1320.9 and the related provisions of 5 CFR 1320.8(b)(3).
