THE INFORMATION COLLECTED ON THESE FORMS WILL BE USED TO VERIFY AN ALLEGATION THAT A RECIPIENT OF SUPPLEMENTAL SECURITY INCOME (SSI) HAS BORROWED MONEY ON AN INFORMAL BASIS.
