Abstract:
THE FORM IS USED TO COLLECT BASIC INFORMATION FROM QUALIFIED INDIVIDUA WHO HAVE BEEN RECOMMENDED FOR PANEL SERVICE. INFORMATION IS ENTERED INTO A COMPUTER DATA BASE WHICH SERVES AS A REFERENCE TOOL FOR ENDOWME STAFF TO AID IN ASSEMBLING ADVISORY PANELS WHICH MEET CONGRESSIONAL REQUIREMENTS FOR BROAD REPRESENTATION.
