The survey instrument and plan for the National Survey of Family Growth, Cycle V are approved for use through 7/96. OMB shares the attached observations for the National Center for Health Statistics' (NCHS) consideration, however. It would be useful for NCHS to address these observations in writing for the public docket.

THE SURVEY PROVIDES DATA ON CHILDBEARING, FAMILY FORMATION (INCLUDING ADOPTION), AND MATERNAL AND CHILD HEALTH. THE DATA ARE USED BY THE OFFICE OF POPULATION AFFAIRS, THE NICHHD, THE CDC, AND OTHER AGENCIES AND ARE DISSEMINATED THROUGH WRITTEN REPORTS AND PUBLIC USE COMPUTER TAPES.
