This Application for the Nursing Education Loan Repayment Program (NELRP) is approved for use through 12/97 with the exception of question F pertaining to race/ethnicity. OMB believes that inclusion of this question could lead to respondents' mistaken impressions that application processing would depend upon racial/ethnic variables. OMB recommends that HRSA collect this information later through separate, sampled survey of applicant universe.

NURSES WITH EDUCATION LOANS USE THE APPLICATION TO APPLY FOR REPAYMENT OF THE LOANS IN RETURN FOR SERVICE IN AN AREA DESIGNATED BY THE SECRETARY AS HAVING A SHORTAGE OF NURSES. FOR NURSES ACCEPTED INTO TH PROGRAM, THEIR LENDERS MUST CONFIRM THE PURPOSE AND AMOUNT OF THE LOAN TO BE REPAID.
