This information collection is approved provided that the Department removes the five-year plan and the annual progress and services report from this collection package and resubmits both under a separate collection. HHS must submit the collection for the State Plan to OMB by 4/15/95. Approval of this package includes only Parts I and II of the CWS-101. HHS agrees to make all of the changes to the form as agreed to with OMB and as reflected in the revised pages sent to OMB on 3/23/95.

In order to receive funds under Title IV-B of the Social Security Act, the NPRM proposes that States and Indian tribes must submit a five-year plan in FY 1995 and annual progress and services reports for FY 1996-1999.
