Approved as amended February 28, 1996 by a memorandum signed by Joy Ddezzuti. The memorandum states that each respondent will be notified by information included in the October and May mailing of this form which provides the disclosure notice required by the 1995 PRA. When the stock of current forms is depleted, the form will be issued with the notice required by the 1995 PRA. BLS should provide OMB with a copy of the notice to be sent in the May and October mailings as soon as it BLS has approved it for use. The OMB number of this docket should be included is ensure proper filing.

Abstract:
These forms are used to collect price information for the Producer Price Index. The Producer Price Index, which is one of the Nation's leading economic indicators, is used as a measure of price movements, indicator of inflationary trends in the economy, inventory valuation measure for some organizations, and measure of purchasing power of the dollar at the primary market level.
