This information collection is approved through 6-98 under the following conditions: As agreed to by FCS, the Agency will modify the sample selection methodology to initially collect information from 480 SFA's to ensure the research attains the necessary number of cases, given the expected non-response rate. FCS will need to re-run the randomized sample within each Region to reflect these terms.
