Abstract:
The form OWCP-16 is the plan for rehabilitation services submitted to OWCP by the injured worker and rehabilitation counselor and OWCP's AWA of payment from funds provided for rehabilitation. The form summarizes the nature and costs of the rehabilitation program for a prompt decision about funding by OWCP to expedite the continuation of the rehabilitation process.
