OMB is approving this collection provided that before the form is circulated to Organ Procurement Organizations: 1.) the editorial corrections noted on the form in HCFA's Paperwork Reduction Act (PRA) submission and the fascimile from OMB to HCFA on 5/20/96 are made; and 2.) the disclosure statements are consistent with the PRA of 1995 and its implementing regulations. HCFA must submit the amended form with disclosures for OMB's public record. This approval will expire in two years to coincide with the 1998 certification/recertification cycle for OPOs, and to ensure consistency with any amendments subsequently made to the final rules published on 5/2/96.

On behalf of this Federal agency, I certify that the collection of information encompassed by this request complies with 5 CFR 1320.9 and the related provisions of 5 CFR 1320.8(b)(3).
The following is a summary of the topics, regarding the proposed collection of information, that the certification covers:
