Terms of Clearance: This approval is for a limited time period. HUD must incorporate all of the information required at 5 CFR 1320.8(b)(3)into the addendum. HUD has failed to include a statement regarding the legal significance of display of the OMB control number. Use of the addendum should be a transitional step before incorporating all of this information into the printed form. In its next request for OMB approval, HUD must show that the required statments are being incorporated into the printed form.
