Abstract:
The VS-10-4 is routinely used when specimens (blood, milk, tissue, or urine) from any animal (including cattle, swine, sheep, horses, or poultry) are submitted to the NVSL for disease testing. The form identifies the herd or flock from which the specimens were taken, the purpose for submitting the specimens, the type of specimen, and the species of animal.
