Terms of Clearance: This collection is approved as amended by OCSE's memorandum of 2/28/97 and on the following conditions: (1) OCSE will send the revised form and instructions to OMB prior to distributing the form to States. (2) In addition to seeking comment through the Federal Register notice as part of the formal clearance process, OCSE shall extensively consult with and obtain feedback from employers and employer organizations, including the Office of Personnel Management (OPM). Any comments received as part of that process shall be submitted to OMB at the time of the next clearance, along with a response to the comments.
