Terms of Clearance: Approved consistent with and as amended by ED's memo of 6/11/98. In addition, ED agrees to the following conditions: 1) This application will be accessible on the web by the end of FY 1998; 2) ED will continue its efforts to provide respondents with the option to transmit this application electronically; 3) ED will use the newly approved version of the AFEA (approved 6/19/98) and therefore will not require respondents to report on Congressional Districts; 4) Due to ED adding the social security number to this form, ED will, within 6 weeks of this approval, begin the procedure for modifying the current system of records through a RONS notice; 5) ED agrees to update the forms to include Privacy Act statements that specify how the social security number will be used by ED.
