Abstract:
Forms used only in Presidentially declared major disasters or emergencies to allow individuals, farmers, small business owners, and private nonprofit organizations to apply for Federal disaster assistance. The information serves as the application for FEMA's Disaster Housing Program and Individual and Family Grant Program and is relayed to other Federal and State agencies administering disaster relief programs appropriate to the applicant's needs.

On behalf of this Federal agency, I certify that the collection of information encompassed by this request complies with 5 CFR 1320.9 and the related provisions of 5 CFR 1320.8(b)(3).
The following is a summary of the topics, regarding the proposed collection of information, that the certification covers:
