This collection is approved, subject to the changes cited in the CDC memos dated 10/21/99 and 10/26/99. In addition, it is very important that CDC raise the rate of response in this study. As agreed upon, CDC will evaluate the rate of response after 1 year. If the response rate is not at least 75 percent, CDC will explore the use of additional incentive payments or other methods of boosting the response rate. At this time CDC will consult with OMB, and resubmit the package for approval if necessary.
