Abstract:
This form is used to apply for a trusteeship to act on behalf of an incompetent member of the uniformed services with regard to military pay matters. The authority to collect this information is found in 37 U.S.C. sections 602-604. The authority to appoint trustees under Title 37 has been delegated to the Director, DFAS-Cleveland Center.
